Arkansas Otolaryngology Center has incorporated electronic patient forms in most scenarios. These questions will be asked throughout your electronic check-in process which will come to you via text at the time of your appointment reminder. Please consider completing this information prior to your appointment time OR if you intend on using a device provided by our clinic plan arrive 20 minutes early.

ALLERGY PATIENT FORMS

All new allergy patients are required to complete a new patient information sheet, a patient questionnaire, and a notice of privacy. Click on the link to print and complete the forms before your visit.
